After you have filled out the hospital’s form to apply for financial assistance and gathered the required documents, it’s time to send your application to the hospital.
Ways to submit your financial assistance application
Each hospital has different rules for how to submit the application, so make sure to read their instructions. You might be able to:
1. Walk into the hospital and deliver it by hand
Some hospitals will provide an address where you can drop off your application. When you get there, try to find the financial assistance (or billing) department and explain to someone there that you are delivering your financial assistance application.
2. Mail it
The application will probably include a mailing address where you can send the application. Be careful to use enough postage. You might want to pay a little extra for certified mail to get proof that the hospital has received it.
3. E-mail it
Some hospitals have an email address where you can send the application. Make sure you attach copies of all of the required documents. You can use your phone to take pictures of each page if needed.
4. Fax it
A lot of hospitals still use faxes to accept applications. If you don’t have access to a fax machine (who does?), you can use an online fax program to send it.
REMEMBER…An application is not complete without:
- An application form from the hospital that you’ve completed and signed
- Your most recent three months of paystubs
- Last year’s tax return
- Any other documents required by hospital
It’s never a bad idea to call your hospital a few days after you send in your application to confirm they have received it – especially if you mailed or faxed it in.
